The Rosacea Puzzle: What's Actually Happening Under Your Skin

If you've ever had someone ask "are you blushing?" when you're not, or watched your face go red after a glass of wine or a hot cup of coffee and stay that way for hours — you already know rosacea isn't just "sensitive skin." It's a chronic inflammatory condition, it's genuinely confusing to live with, and honestly, most of what's out there about it is either outdated or oversimplified. So let's dig into what's actually going on, and where it fits into the same inside-out philosophy I talk about constantly around here.

What Rosacea Actually Is

Rosacea is a chronic condition centered on the cheeks, nose, chin, and forehead that generally shows up as one or more of: persistent redness, visible blood vessels, flushing/blushing episodes that don't fully fade, and sometimes small red bumps or pus-filled lesions that mimic acne. There's an ocular form too, where the eyes get dry, gritty, and irritated. It's chronic — meaning it tends to wax and wane rather than resolve — and it commonly starts showing up in the 30s through 50s, more often in people with fair skin, though it affects a wide range of skin tones and is frequently under-recognized in those cases.

For a long time rosacea got filed under "just a skin thing." The research over the last few years has moved well past that.

What's Actually Causing It

This is where it gets genuinely interesting, and where the science has shifted a lot recently.

It's not one thing — it's several overlapping systems misfiring together. Current research describes rosacea as a multifactorial condition involving genetic predisposition, an overactive innate immune response, blood vessel and nerve signaling gone haywire, and environmental exposures all layered on top of each other.

Demodex mites are a much bigger piece of the puzzle than we used to think. Nearly every adult has these microscopic mites living in their facial pores — that's normal and not a hygiene issue. But research increasingly shows that rosacea patients tend to have mite populations that expand well beyond typical levels, and at that higher density the mites appear to overwhelm the skin's normal immune tolerance and trigger ongoing inflammation. It's not that the mites are present — it's that there are too many of them, and some studies suggest the bacteria mites carry may add fuel to the fire.

The gut-skin connection is real here too, and it's not just a wellness buzzword. A growing body of research frames rosacea through what's being called the skin-gut-brain axis — looking at rosacea as involving dysregulated interactions between the skin, the gastrointestinal system, and the central nervous system, rather than as an isolated dermatological problem. Gut microbiome imbalances, small intestinal bacterial overgrowth (SIBO), and even H. pylori infection have all been studied for their possible role in driving rosacea flares, though researchers are careful to note that more work is needed to fully understand the mechanisms. This tracks with everything I talk about regarding the gut-skin axis for skin health generally — your face is often reporting on what's happening several feet south of it.

Blood vessels and nerves are oversensitized. Rosacea-prone skin has a lower threshold for triggering flushing — heat, certain foods, stress, and other stimuli activate nerve pathways and blood vessel dilation more easily and more persistently than in typical skin.

Age and Hormones

Rosacea most commonly develops between the 30s and 50s, and there's a well-documented connection to hormonal transition — rosacea is more frequently reported around perimenopause and menopause. That doesn't mean hormones "cause" rosacea on their own, but the hormonal shifts of this life stage do seem to lower the threshold for flares in people already predisposed. If you've read my post on the hormone conversation nobody has with you, this is one more example of how much skin behavior is downstream of what's happening hormonally — not just what's sitting in your medicine cabinet.

Sun Exposure

Sun is consistently named as the single most common rosacea trigger. UV exposure ramps up inflammatory signaling and blood vessel activity in skin that's already prone to overreacting. The standard advice holds here: seek shade, wear a wide-brimmed hat, avoid peak sun hours, and use a clean mineral sunscreen on affected areas.

Heat in almost any form is a recurring theme: hot showers, saunas, steam rooms, intense exercise, and overheated rooms can all set off flushing. So can big swings in temperature — walking from a cold parking lot into an overheated store, for example. Emotional stress is a major one too; stress activates inflammatory pathways that directly worsen redness and sensitivity, which is part of why stress management (sleep, movement, breathing practices, whatever actually works for you) isn't a "nice to have" for rosacea, it's part of the management plan. Certain medications are worth knowing about as well — steroid creams, some blood pressure and migraine medications, and niacin supplements can all trigger flushing, so it's worth a conversation with your prescriber if you notice a pattern.

Food and Drink

This is the trigger category people ask about most, and it's genuinely individual — there isn't strong evidence for one universal "rosacea diet," but there's a well-established pattern of common culprits:

  • Alcohol, especially red wine, dilates blood vessels and is one of the most consistently reported triggers
  • Hot beverages — the temperature itself, not necessarily the ingredients, is often the trigger. Letting your coffee or tea cool a bit, or switching to iced versions, can make a real difference
  • Spicy foods containing capsaicin (chilies, hot sauce) or cinnamaldehyde (which shows up in cinnamon, tomatoes, chocolate, and citrus) raise skin temperature and blood flow
  • Some dairy products are reported as triggers for a subset of people
  • Niacin-rich foods and supplements can cause flushing even in people without rosacea, and more so in people who have it (this is a B vitamin and necessary for human life. You do want to make sure you are getting enough niacin, and flush-free options are available if supplementation is needed)

Because of the gut-skin axis piece, some people also find that broader gut-supportive eating (fiber, fermented foods, minimizing ultra-processed food) makes a difference over time, even outside of any specific "trigger food" — though this is more individual experimentation than settled science. Keeping a simple flare journal (what you ate, drank, or did in the 24 hours before a flare) is genuinely one of the most useful low-tech tools for figuring out your personal pattern.

Washing and Product Choice

This is where a lot of well-meaning skincare routines accidentally make rosacea worse. Rosacea-prone skin has a compromised barrier, meaning it's more permeable and more reactive to begin with — so the goal is to do less, more gently, not more, more aggressively.

A few things worth knowing:

  • Fragrance is one of the most common irritant triggers, including "natural" fragrance from essential oils — a rosacea-reactive face is not the place to experiment with a strongly scented product, even a clean one
  • Physical scrubs, exfoliating acids, and harsh cleansers disrupt an already-compromised barrier further
  • Alcohol-based toners and astringents dry and irritate reactive skin
  • Menthol, camphor, and very "cooling" or "tingly" ingredients can actually trigger flushing rather than calm it, despite feeling soothing in the moment
  • Lukewarm water, not hot, for washing your face — hot water is its own trigger
  • Consistency matters more than intensity — a simple, gentle, barrier-supportive routine done every day tends to outperform an aggressive routine done sporadically

What You Might Not Have Thought to Ask

A few things that don't always make it into the standard rosacea explainer:

  • Ocular rosacea is real and easy to miss. Dry, gritty, red, or irritated eyes alongside facial redness can be an eye-involvement piece of rosacea — worth mentioning to your dermatologist or ophthalmologist rather than just reaching for allergy eye drops.
  • Rosacea is frequently misdiagnosed as adult acne, which leads people toward acne treatments (often harsh, drying, or exfoliating) that can actively worsen rosacea. If "acne treatment" isn't working and your skin looks angrier the more you treat it, it's worth asking whether rosacea is actually the underlying picture.
  • Photo tracking helps more than memory does. Taking a simple photo of your face during a flare and during a calm period, over time, makes patterns and triggers much easier to spot than trying to remember what you ate three days ago.
  • It's chronic, not something you "cure" and move on from — the realistic goal is management and fewer, milder flares, not permanent elimination. That's not a discouraging framing, it's just an honest one, and it takes the pressure off chasing a "fix."

Where My Formula Fits In

I want to be careful and honest with you here: nothing I make treats or cures rosacea, and I'm not going to pretend otherwise — that's a job for your dermatologist and, where appropriate, prescription treatment. What I can speak to is the barrier-support piece, which matters a lot given how central barrier disruption is to rosacea-prone skin.

Grass-fed tallow is structurally close to what your own skin already produces, which is part of why it tends to sit well on reactive, barrier-compromised skin rather than sitting on top of it or clogging it. For rosacea specifically, here's how I'd think about it:

  • No Bull (Unscented) is the one I'd point you toward first. No essential oils at all means one less category of potential irritant on skin that's already prone to overreacting — this is the same logic I use when I talk about eczema-prone or baby skin.
  • Skip the scented variants if your skin is actively reactive, at least until you know how your particular rosacea responds to fragrance. Even the "cleanest" essential oil blends I use are still essential oils, and rosacea skin can be a harder crowd to please than typical skin.
  • The formula is simple on purpose — tallow, castor oil, sunflower oil, beeswax, and arrowroot powder. No synthetic fragrance, no drying alcohols, nothing designed to "tingle" or foam aggressively. That's the barrier-friendly, do-less-but-better philosophy rosacea-prone skin generally responds better to.
  • Apply to clean, lukewarm-washed skin rather than after a hot shower or hot-water cleanse, for the reasons above.

Think of it less as a rosacea treatment and more as one gentle, low-irritant piece of a bigger picture that includes sun protection, trigger tracking, dermatologist care, and — per everything above — paying attention to what's happening well below the surface of your skin.

A Simple Action Plan

If you're newly noticing rosacea-like symptoms or trying to get a handle on existing flares:

  1. Get a diagnosis. Rosacea mimics acne and a few other conditions, so start with a dermatologist rather than guessing.
  2. Start a simple flare journal — food, drink, temperature, stress, sun exposure, skincare products used, for a couple of weeks. Patterns show up faster than you'd expect.
  3. Switch to lukewarm water and a fragrance-free routine while you're figuring out your personal triggers.
  4. Get serious about daily sun protection with a properly tested broad-spectrum mineral SPF.
  5. Don't skip the gut piece — sleep, stress management, and general gut-supportive eating aren't a separate wellness project from your skin, they're part of the same system.
  6. Bring your journal to your dermatologist appointment — it makes the conversation about medical treatment options far more productive.
