At the market, I'll spend twenty minutes with someone talking about gut health, seed oils, glyphosate, hormones — all of it. And then almost as an afterthought, they'll mention they've been averaging five hours of sleep a night for years. Nobody thinks to lead with that. Sleep has become the thing we sacrifice first and mention last.

That's backwards. Of everything we can control about our health, sleep may be the single most powerful lever — and it's the one most of us have quietly stopped pulling.

Think about how much airtime diet gets. Exercise. Supplements. Skincare ingredients. Entire industries exist around each of those, and rightfully so — we've spent a lot of time on this blog in that territory ourselves. But sleep rarely gets mentioned in the same breath, and when it does, it's treated as a nice-to-have rather than as the thing everything else is built on top of. That's a mistake. You can eat a spotless diet, take every supplement, use the cleanest skincare on the market, and still undercut all of it with six hours of fragmented sleep a night. Sleep isn't one input among many. It's closer to the operating system the other inputs run on.

WHY WE STOPPED TAKING SLEEP SERIOUSLY

Somewhere along the way, needing sleep became a personal failing instead of a biological requirement. "I'll sleep when I'm dead" became a badge of honor. We optimize our diets, our workouts, our skincare routines — and then stay up until midnight scrolling, three coffees deep, wondering why none of it seems to be working the way it should.

Here's the thing: sleep isn't downtime. It isn't your body doing nothing. It's one of the most metabolically active, tightly regulated processes you go through every day — and almost every system we've discussed on this blog runs through it. Your gut, your skin, your hormones, your mood, your inflammation levels. Sleep is the thread connecting all of it.

WHAT'S ACTUALLY HAPPENING WHILE YOU SLEEP

While you're unconscious, your body isn't idle — it's running maintenance.

Growth hormone (GH) is released in pulses throughout the night, with the majority — research points to roughly 70-80% of daily GH secretion in children — occurring during slow-wave (deep) sleep. GH drives tissue repair, muscle growth, bone density, and — importantly for our purposes — collagen production in the skin. This is not a minor process happening in the background. It's one of the primary repair windows your body gets all day.

Your brain does its own overnight cleaning. During sleep, the brain's glymphatic system ramps up, flushing out metabolic waste — including proteins linked to cognitive decline — that accumulate during waking hours. This is one reason chronic poor sleep is so strongly tied to brain fog and, over the long term, cognitive risk.

Cortisol and melatonin trade places. In a healthy circadian rhythm, cortisol should be low in the evening and rise toward morning, while melatonin rises at night and falls with daylight. This isn't just about feeling sleepy — cortisol is a stress hormone, and when it stays elevated because your rhythm is disrupted, it actively works against nearly everything we care about here: gut lining integrity, immune regulation, and — as we'll get to — collagen.

Your immune system recalibrates. Sleep supports the production and function of the immune cells that fight off illness. This is why you're more susceptible to getting sick when you're running on empty, and why illness makes you want to sleep more — your body is asking for the resource it needs most.

THE SLEEP WRECKERS: WHAT'S QUIETLY RUINING YOUR NIGHT

Most people aren't choosing bad sleep. They're unknowingly sabotaging it, often with things that feel harmless or even helpful in the moment.

Alcohol. The nightcap is one of the biggest sleep myths out there. Alcohol may help you fall asleep faster, but it fragments sleep architecture later in the night, suppressing REM sleep and causing you to wake more often — even if you don't remember waking. You can log eight hours and still feel like you got four.

Caffeine. Caffeine has a half-life of roughly five to six hours, meaning that 2pm coffee still has a meaningful amount circulating in your system at 8pm. For sensitive individuals, that's enough to delay sleep onset and reduce deep sleep, even if you don't consciously feel "wired" at bedtime.

Sugar and blood sugar swings. A high-sugar dinner or dessert can send your blood sugar climbing, followed by a crash in the middle of the night. That crash triggers a cortisol and adrenaline response to bring your blood sugar back up — which is a very effective way to wake yourself at 3am with your heart pounding for no obvious reason.

Eating too late. Digestion is an active metabolic process, and lying down to digest a full meal works against your body's natural drop in core temperature that helps trigger sleep onset. Late, heavy meals are also more likely to contribute to reflux, which disrupts sleep directly.

Gut health. We've talked before about how the gut produces the majority of the body's serotonin — the precursor to melatonin. A disrupted, dysbiotic gut doesn't just affect digestion; it can directly impair your ability to produce the hormones that regulate sleep. This is the gut-brain axis in action, and it runs in both directions: poor sleep also disrupts the gut microbiome, which can make sleep worse. It's a loop, not a one-way street.

Stress and cortisol. Chronic stress keeps cortisol elevated when it should be winding down in the evening. This is why so many people report being "tired but wired" at bedtime — exhausted, but unable to actually fall asleep, because their nervous system hasn't gotten the signal that it's safe to power down.

Screens and blue light. This one's almost too obvious to mention, and yet it's probably the single most common modern sleep disruptor. Blue light exposure in the evening suppresses melatonin production, delaying your body's natural signal that it's time to wind down.

THE HORMONE PIECE MOST WOMEN AREN'T TOLD ABOUT

If you're a woman in your late 30s, 40s, or 50s reading this, there's a piece of the sleep puzzle that deserves its own section, because it rarely gets mentioned outside of a specialist's office: progesterone.

Progesterone isn't just a reproductive hormone. It also acts directly on the brain, where it has a natural calming, sedative-like effect — similar in feel to how a sleep medication works, except your body makes it on its own. This is part of why many women sleep noticeably better during the second half of their menstrual cycle, when progesterone is highest, and why sleep so often falls apart during perimenopause, when progesterone is typically the first hormone to decline — often years before estrogen drops significantly and well before a woman would think to call herself "menopausal."

This decline is a real, measurable driver of the insomnia, 3am waking, and night sweats so many women in perimenopause experience. It's not just stress. It's not just "getting older." There's a specific hormonal reason behind it, and that matters, because it means it's something that can potentially be addressed rather than simply endured.

What the research shows. Multiple clinical studies now back this up. A review pooling results from randomized trials found that bioidentical progesterone — meaning it's structurally identical to what your own body makes, taken in a form your body can use — measurably improves sleep quality in postmenopausal women. In one well-designed trial, women taking progesterone at bedtime reported significantly better sleep and fewer night sweats than those taking a placebo, with no safety concerns. Other research has found that progesterone increases deep sleep and helps regulate the same nighttime hormone processes we talked about earlier in this post — growth hormone and melatonin.

One especially interesting finding: progesterone doesn't seem to sedate or change sleep that's already going fine. It specifically seems to help restore sleep once something has disrupted it — which is exactly the pattern so many perimenopausal women describe.

An important distinction. Bioidentical progesterone is not the same thing as the synthetic progestins used in some older hormone therapy formulations, which come with a different — and more debated — risk profile. This is a meaningful distinction, and one worth understanding before any conversation with your provider.

This is genuinely a conversation to have with a doctor, not a DIY decision. Hormone therapy is individualized — it depends on your full health picture, history, and factors only a qualified provider can properly evaluate. But if you're a woman whose sleep has quietly fallen apart somewhere in your late 30s, 40s, or 50s and you've been told it's simply stress or aging, it's worth knowing there's real, published research behind a hormonal explanation — and that it's a completely legitimate thing to bring up with your doctor, OB-GYN, or a menopause-informed provider, rather than something to just push through.

WHAT POOR SLEEP DOES TO YOU

The downstream effects of chronic poor sleep touch nearly every system in the body:

  • Mood. Sleep deprivation is strongly linked to increased anxiety, irritability, and depressive symptoms. This isn't a coincidence — many of the same neurotransmitters affected by gut health (serotonin, dopamine, GABA) are also regulated by sleep.
  • Inflammation. Poor sleep raises circulating inflammatory markers throughout the body, which shows up everywhere from joint pain to skin flares to cardiovascular risk.
  • Immune function. As mentioned above, chronic sleep restriction measurably reduces your ability to fight off illness.
  • Weight and metabolism. Sleep regulates leptin and ghrelin — your satiety and hunger hormones. When you're sleep-deprived, ghrelin rises and leptin falls, which means you feel hungrier and less satisfied, and tend to crave exactly the sugary, high-carb foods that disrupt sleep even further. It's the same loop as the gut connection above, just with a different hormone pair.
  • Cognitive function. Memory consolidation happens during sleep. Chronic poor sleep measurably impairs learning, focus, and decision-making.

A SPECIAL NOTE ON BABIES, CHILDREN, AND TEENS

I want to spend real time here, because I think this is one of the most misunderstood areas of sleep — and one where well-meaning advice can actually work against a baby's biology.

Sleep is when children grow. The same growth hormone process we described above is even more pronounced in children — research points to a substantial majority of daily GH secretion in children happening during deep sleep, driving the bone growth, muscle development, and tissue repair of an actively growing body. Sleep isn't just rest for a baby or toddler. It's construction time.

"Sleeping through the night" is a myth we need to retire. Infants — especially breastfed infants — are not biologically designed to sleep eight uninterrupted hours. Human breast milk is digested quickly, and babies are in an extreme state of growth that requires frequent nourishment, often every two to three hours, including overnight, for months. A baby waking at night isn't a sign that something is wrong or that you're doing something wrong. It's the system working exactly as designed.

This matters because so much pressure gets placed on parents — often starting in the earliest weeks — to get a baby to "sleep through the night," as though that's the milestone that signals success. It isn't. It's a modern expectation layered onto an ancient, well-designed biological process.

Rice cereal in the bottle is a myth worth retiring too. This is genuinely one of the oldest pieces of "sleep advice" out there — thickening a baby's bottle with rice cereal to fill them up and buy a few extra hours of sleep. The American Academy of Pediatrics recommends against this practice. It doesn't have solid evidence behind it as a sleep aid, it's a choking hazard for infants who aren't developmentally ready for thicker textures, and it's been associated with excessive weight gain and constipation. Babies regulate their intake remarkably well across a 24-hour period — the goal isn't to override that system, it's to trust it.

Solid foods introduced too early can seed the gut in the wrong direction. We spent a lot of time in our gut post talking about the first years of life as a critical window for microbiome development. The research bears this out directly: the first roughly 1,000 days of life are considered the key window for gut microbiota modulation, and introducing solid foods before a baby's gut and immune system are developmentally ready — generally before four to six months — has been associated with increased intestinal permeability and a higher risk of food sensitivities down the line. This lines up with everything we know about leaky gut in adults; it's the same mechanism, just showing up at the very start of life, when the gut lining is still maturing. The tongue-thrust reflex and swallowing coordination babies need to safely handle solids typically aren't in place until around four to six months for a reason — it's not an arbitrary cutoff.

A note for the parents, not just the babies. None of this is said to add guilt to an already exhausting season. Frequent night waking is normal — but it's also genuinely, brutally hard on the adult doing the waking up. If you're in the thick of it, your own sleep matters too, and it's worth leaning on any support you have access to, rather than reaching for a workaround that fights your baby's biology instead of working with it.

And don't forget the teenagers. It's easy to assume sleep concerns taper off once a child is past the toddler years, but teenagers are arguably one of the most sleep-deprived groups out there, and it's not really their fault. Both the National Sleep Foundation and the American Academy of Sleep Medicine recommend 8 to 10 hours a night for teenagers — more than most adults need, because the adolescent brain is in an intensive period of development and reorganization. But puberty triggers a genuine, well-documented shift in circadian rhythm called "sleep phase delay," pushing natural melatonin release one to three hours later than in childhood. A teenager's brain often doesn't start producing sleep-promoting melatonin until 10 or 11pm, regardless of how early they're told to go to bed. That's not defiance or laziness — it's biology working against the school bell. Combine an early school start time with a body that's biologically wired to stay up later, and most teens are running a structural sleep deficit that has nothing to do with discipline. Given everything we've covered in this post — growth hormone, mood regulation, inflammation, cognitive function — a chronically under-slept teenager is dealing with all of it during some of the most demanding developmental years of their life. Where you have influence, protecting even an extra 30-60 minutes of sleep at the edges, and being thoughtful about screens and late-night stimulation, can meaningfully help.

SLEEP AND YOUR SKIN: THE VISIBLE TOLL

This is the part that tends to get people's attention, because unlike inflammation or gut health, this one shows up in the mirror.

Dark circles are largely a circulation and fluid issue — poor sleep contributes to blood pooling and fluid retention under the thin skin of the eye area, making underlying blood vessels more visible.

Collagen production and breakdown are both affected — in opposite directions. We mentioned above that growth hormone drives collagen synthesis during deep sleep. At the same time, poor sleep elevates cortisol, and cortisol actively suppresses the fibroblast activity responsible for producing collagen and elastin, while triggering enzymes that break existing collagen down. That's a double hit: less being built, more being broken down. One recent review comparing skin aging scores between good and poor sleepers found that poor sleepers showed nearly double the visible signs of intrinsic aging — fine lines, uneven pigmentation, reduced elasticity — compared to good sleepers, independent of sun exposure.

Wrinkles and sagging follow directly from that collagen math. Less collagen production, more collagen breakdown, repeated night after night, is a slow but steady path toward thinner, less elastic, more lined skin.

Your skin barrier itself repairs overnight. Skin operates on its own circadian rhythm — more defensive during the day, more regenerative at night. Poor sleep has been shown to increase water loss through the skin and slow the skin's recovery from daily stressors like UV exposure.

THE LEVERAGE POINT: WHY SLEEP MIGHT BE WHERE TO START

If you've read this far, you've probably noticed something: sleep doesn't show up in just one section of your health. It shows up in every single one we've covered on this blog.

It's in the gut post — sleep and the microbiome affect each other in both directions. It's in the inflammation conversation — poor sleep raises inflammatory markers throughout the body. It's in the hormone story — cortisol, melatonin, leptin, ghrelin, growth hormone, progesterone, even estrogen metabolism, all regulated in part by sleep. It's in the skin conversation, obviously. It's even in willpower and decision-making — it's a lot harder to choose the salad over the drive-thru, or to skip the second glass of wine, when you're running on four hours of sleep and your body is chemically begging for quick energy.

This is why I think sleep deserves to be a starting point rather than an afterthought. Most people try to fix their health by adding things — a new supplement, a new diet, a new skincare routine — while the foundation underneath all of it stays cracked. Fixing sleep first doesn't replace any of that other work. It makes all of it work better.

Here's what tends to happen when someone genuinely prioritizes sleep for a few weeks, rather than just wishing for more of it: cravings for sugar and refined carbs quiet down, because leptin and ghrelin are finally regulated instead of working against you. Stress feels more manageable, because cortisol isn't already elevated before the day's actual stressors show up. Gut symptoms often improve, because the gut-brain axis is finally getting the signal it needs to regulate itself. Skin looks less inflamed and more rested, quite literally. And — maybe most underrated of all — the motivation to make other good choices tends to come back on its own, because a well-rested brain has more capacity for the executive function that healthy choices require.

None of this means sleep is a cure-all, and it's not a reason to ignore the other pieces we've written about here. But if you're standing at the start of a health journey and feeling overwhelmed by everything you're "supposed" to fix at once, sleep is a reasonable place to put your first real effort. It's the one change that quietly makes every other change easier.

THE ACTION PLAN

This isn't medical advice — it's a starting framework. Work with a healthcare provider for anything persistent or severe.

Step 1: Protect your first sleep cycle. The first stretch of deep sleep after you fall asleep is when the largest growth hormone pulse occurs. Going to bed even 30 minutes earlier can protect this window.

Step 2: Cut caffeine off by early afternoon. Given caffeine's five-to-six-hour half-life, an early cutoff gives your body a real chance to clear it before bed.

Step 3: Skip the nightcap. If you drink, try to finish at least a few hours before bed so alcohol has time to clear before it can fragment your sleep architecture.

Step 4: Stop eating two to three hours before bed. This gives digestion time to settle and lets your core temperature drop naturally, which helps trigger sleep onset.

Step 5: Support your gut. Everything we've written about the gut-skin axis applies here directly — a well-supported gut is better equipped to produce the serotonin your body needs to make melatonin.

Step 6: Get morning light. Exposure to natural light early in the day helps anchor your circadian rhythm, making it easier for melatonin to rise on schedule at night.

Step 7: Put the screens away before bed. Even 30-60 minutes of a dark, screen-free wind-down can make a meaningful difference in melatonin production.

Step 8: For babies — feed on demand, skip the cereal, and let solids wait for real readiness signs. Trust the frequent waking as biology doing its job, not a problem to be solved.

WHAT YOU PUT ON YOUR SKIN STILL MATTERS

None of this means skincare is beside the point — it means skincare works best when it's supporting a body that's already doing its own repair work well.
